Starting with no identity documents?

A certified medical record may help you begin. Ask a clinic, doctor, or hospital for a record or letter certified by the medical-record custodian that shows your name and date of birth or age. A receipt, an uncertified patient copy, or an adult immunization record is not the same thing.

What a provider letter can do

A shelter or service provider can help establish Texas residency by completing a notarized residency affidavit or letter and receiving mail for you. A general homelessness letter does not, by itself, prove identity to Social Security or DPS.

Do you have a mugshot or booking photo?

Some local ID-recovery providers may accept a mugshot or booking photo as a one-time bridge document to help recover a stronger record. It is not a reusable government ID and is not guaranteed to be accepted directly by Social Security, Texas Vital Statistics, or DPS. Confirm the provider’s current policy before relying on it.

STEP 1

Get a replacement Social Security card

This creates an important supporting document for the next steps.

  • Use stronger proof of identity if you have it.
  • If you do not, ask whether SSA can accept your certified medical record or a certified medical-record extract showing your name and date of birth or age.
  • Bring original documents or copies certified by the issuing agency. Photocopies and notarized copies are not accepted.
  • A replacement Social Security card is free.

STEP 2

Order your Texas birth certificate

Your certified birth certificate becomes identity and citizenship evidence for a Texas ID.

A signed Social Security card alone is not enough. Texas requires one of these combinations:

  • 1 document from Group A, or
  • 2 documents from Group B, or
  • 1 document from Group B plus 2 documents from Group C.

Your signed Social Security card is Group B. Other records may include an expired photo ID, Medicaid or Medicare card, insurance card, employment ID, public-assistance letter, school transcript, bank statement, library card, or recent medical record or bill. A mugshot is not listed as a standard Texas Vital Statistics ID document, so use it through a provider only when that provider confirms it can serve as a one-time bridge. Check the official list before going.

STEP 3

Apply for a Texas identification card

This gives you the photo ID commonly needed for work, housing, banking, and other goals.

You will need evidence of identity, U.S. citizenship or lawful presence, Social Security number, and Texas residency.

  • A certified birth certificate counts as a secondary identity document.
  • A Social Security card is a supporting identity document. You generally need another supporting document with the birth certificate.
  • If you lack regular address documents, a qualifying government, nonprofit, shelter, transitional-services, or group-home representative may complete a notarized DL-5 or residency letter for the address where you live or receive services and mail.
  • Schedule a DPS appointment and take every document you have.
